Card Application

| 1. | Are all fields on the application form compulsory? |
| NO. Only the 'Title; First Name; and Surname' are mandatory, and even 'Title' will not appear on the card as it is not deemed neccessary by emergency services.
We want you to include only the data that you consider appropriate. The more you fill in - the better it can serve you in an emergency - but the choice is yours! |

| 2. | If I leave a field blank - what will appear on the card? |
| Nothing. |

| 3. | Do I need to have a photo on my card? |
| No. All data contained on the card is selected by you. If you don't want a photo, simply don't include one. You can always order a duplicate card later and add a photo at that time. |

| 4. | I don't have anyone personal to contact in case of my emergency - what shall I do? |
| We believe that in such circumstances, at least your lawyer should be informed. Just add him/her under the category: 'Specialist'. |
